It's typically installed in overlapping sections along the eaves and rakes (the sloped edges), with each new piece overlapping the last by a couple of inches to keep water from finding a gap.

Ice and water shield is a self-adhering, waterproof membrane installed underneath the primary underlayment in the areas most vulnerable to water intrusion — eaves, valleys, and around roof penetrations like chimneys and vent pipes. In colder climates, it specifically protects against ice dams: when melting snow refreezes at the roof edge and backs water up underneath the shingles, ice and water shield is often the only thing standing between that backed-up water and your ceiling.

Starter shingles are installed along the eaves (and sometimes the gable ends) before the first visible course of shingles goes on. You'll never actually see them once the job is finished — they sit underneath the first row of field shingles and provide a sealed edge that keeps wind from getting underneath the roof's leading edge and lifting shingles over time.
Because they're invisible in the finished product, starter shingles are one of the easiest line items for a corner-cutting estimate to quietly skip. If they're missing from a bid, it's a completely fair question to ask directly — a contractor who's genuinely including them will be able to tell you exactly where.
Ventilation
Proper roof ventilation depends on a balance between intake (usually through soffit vents at the eaves) and exhaust (typically ridge vent, box vents, or turbine vents along the upper portion of the roof).
